Subject Access Request

Under General Data Protection Regulations, you have the right to access the personal data that the Childhood Eye Cancer Trust (CHECT) holds about you. This is called a Subject Access Request (SAR). To help us identify exactly which information you require, and the format in which you would like to receive it, please complete the form below.

An SAR can be made via a third party. In this case, the third party is required to provide evidence that they are entitled to act in this way (ie written authorisation from the individual concerned).
